CARRIER PACKET WELCOME CHECKLIST

To satisfy our fleet insurance underwriters and maintain our active status with premium expedited broker boards on RMIS and MyCarrierPackets, our compliance department enforces a **Premium Carrier Standard**. Before your first dispatch, we must receive, verify, and store the following active compliance files. This professional standard protects our shared operating authority and secures access to top-paying loads.

PART A: DRIVER COMPLIANCE DOCUMENTS

Submit high-quality PDF files or highly legible smartphone photos.

• Valid Driver's License: A clean, legible copy of your active, standard state driver's license (Class D). Note: No commercial driver's license (CDL) is required for vehicles under 10,010 lbs. GVWR.
• Mandatory Background Check (MVR & Criminal Screening): We will pull your official 3-year Motor Vehicle Record (MVR) driving history and run a criminal background screening upon receipt of your signed authorization form to verify safety status and insurability.
• Functioning Smartphone with Active Data Plan: Required for dispatch communications, real-time GPS tracking apps (MacroPoint, Project44, etc.) during active loads, and uploading signed Bill of Lading (BOL) document photos.
• 🚨 Mandatory Dispatch Communication & Phone Availability: Acknowledgment that when asking dispatch to find/book a load OR while actively carrying a broker's shipment, you MUST keep your phone volume up and remain 100% reachable. Unresponsiveness while asking for freight or in transit is completely unacceptable.

PART B: LEASED VEHICLE DOCUMENTS

• Vehicle Registration: A valid copy of the active commercial vehicle registration card matching the leased vehicle's VIN.
• Mandatory Cargo Securing Equipment: Every leased vehicle must maintain a minimum of four (4) heavy-duty commercial ratchet straps, OR cargo load bars, strapping systems, or cargo netting (E-track systems or heavy-duty anchor D-rings are highly recommended). (Note: While you can lease on without these, lacking them will restrict your dispatch strictly to small boxed cargo that doesn't require strapping, which limits your available loads).
• Photos of the Vehicle: High-resolution photos of all four sides of the vehicle, showing a clean, professional, undamaged exterior (sprinter/high-roof cargo van, 2014 or newer).
• 📐 Inside & Door Opening Dimensions: Grab a tape measure and record your exact vehicle cargo space measurements (cargo floor length, max width, width between wheel wells, rear and side door opening height and width) to fill in Section 1.1 of your Lease Agreement. This is required for our dispatchers to match you with high-paying expedited loads!

PART C: BUSINESS & SETTLEMENT DOCUMENTS

• Signed Independent Contractor Lease Agreement: Completed and executed by both parties.
• Form W-9: Signed W-9 matching your corporate business entity (LLC, S-Corp, or Sole Proprietorship) for year-end tax reporting.
• Payment Preference (Zelle / Voided Check for ACH): Our preferred method is Zelle Instant Payout (fastest & smoothest — funds transfer instantly upon POD approval with an official 1099 paystub sent to your email). Alternatively, you may provide a Voided Check or NOA for direct ACH transfer (takes longer depending on bank cutoff hours).
• Insurance Documents (Select Your Pathway):
  • Pathway A (Scheduled Fleet Auto): Provide a Certificate of Insurance (COI) for Non-Trucking Liability (Bobtail) listing VanHaulers Co as a Certificate Holder, and for Physical Damage listing VanHaulers Co as a Loss Payee & Certificate Holder.
  • Pathway B (Contractor-Provided Insurance):
    • Provide COI showing at least $1,000,000 Primary Commercial Auto Liability and $100,000 Cargo Insurance.
    • The COI must list VanHaulers Co as an Additional Insured (on Auto Liability) and as a Loss Payee & Certificate Holder (on both Auto and Cargo lines) with a 30-day notice of cancellation.
    • Anti-Fraud Submission: The COI must be emailed directly from your licensed insurance agent to recruit@vanhaulers.com (driver-submitted PDFs are invalid).
    • Agent Certification: Your licensed agent must sign and return the interactive Insurance Agent Certification Form.
